Instructor-Led Project Resources

1. Academic Service-Learning Faculty Manual

This manual can help you develop, implement, and evalute service-learning projects in your courses.

2. Faculty Project Checklist

This checklist is designed to assist the Project Facilitor and students in keeping track of the project documentation process.  It assigns documentation responsibilities to both parties.  This form does not need to be turned in to the Center for Teaching and Learning.

3. Project Time Frame

This document is to provide the Project Faciliator with a time frame or structure to complete the service-learing project documentation process.  It includes the general guidelines for project documentation events.  This form does not need to be turned in to the Center for Teaching and Learning.

4. Project Facilitator Agreement

All faculty interested in sponsoring, facilitating, or advising a service-learning project should fill out the Project Facilitator Agreement.  This form may be completed and submitted electronically or in hard copy.

5. Service-Learning Project Proposal

To ensure the success of the project, we ask that all Project Facilitators turn in a Project Proposal.  The purpose of this document is to assist the Project Facilitator in conceptualizing and organizing the project in a manner that will lead to a successful project and rewarding learning experience for the students. This may be submitted electronically or in hard copy.

5a. Budget Proposal

If you will be receiving funds through our office to support a project, we ask that you turn in a Budget Proposal.  The purpose of this document is to provide us with a general idea of how and when the funds will be used.  Securing funds can, at times, be a lengthy process and we wish to make the financial aspects of the project leadership as easy as possible.  This form does need to be turned in to the Center for Teaching and Learning if you are receiving funding through our office.

6. Student Release Form

This is the release form that all students participating in an academic service-learning project must sign before they will be allowed to participate in the project.  We ask that these forms be submitted in hard copy to The Center for Teaching and Learning in the beginning of the semester.

7. Reflection Activities

To assist in the further development of resources as well as to document the student learning that takes place via the service-learning project, we ask that Project Facilitators turn in a response to the Reflection Activities prompt.  This document may be completed and submitted electronically or in hard copy.

8. Service-Learning Project Review Prompt

To ensure the success of future projects, we ask that all Project Facilitators turn in a Project Review.  The purpose of this document is to assist the Service-Learning Coordinator in developing and implementing future project as well as provide the Facilitator and opportunity to reflect on the success of the project.  This document may be completed and submitted electronically or in hard copy.

8a. Budget Report

Service-Learning project require money in order to carry them out.  We recognize the need for financial support and are interested in tracking how much funding different types of projects actually used to be accomplished.   For this reason, we ask that all service-learning projects, whether funded through the CTL or not, turn in a budget report.  This form may be completed and submitted electronically or in hard copy.
